Florida school enrollment starts with health paperwork
Florida school enrollment usually means lining up the local district forms with health exam and immunization paperwork.
For a family move, school enrollment is partly a district errand and partly a health-record errand.
Florida’s health page points to a school-entry health exam for a first entry into Florida pre-K through grade 12. The exam generally needs to be done within one year before enrollment. For students coming from another state, a similar form may work if it fits the timing and provider rules.
Immunization records are a separate part of the folder. Florida uses Form DH 680 for school shots. A Florida doctor, advanced practice registered nurse, or county health department completes that form.
Start with the school district for address, grade, and enrollment forms. Then check the health exam and shot record before the first day gets close. If a record is missing or from another state, ask the county health department or the district how they want it handled. It is easier to fix the folder before the school office is busy.
