Florida has no single statewide 911-address application; the responsible office, documents, and order of steps depend on the property's local jurisdiction.
Florida does not have one statewide form for a new 911 address. The county or city decides which office assigns the address, what it needs, and when the work happens.
That local choice matters for a rural lot, new driveway, split parcel, private road, or new home. The parcel number may exist before the street address. A mailing address, utility account, driveway point, and emergency address may also become ready at different times.
Start with the office that handles the exact property. The job may sit with 911 staff, map staff, planning, building, or another local team. The state list can help you find a county contact, but it is not the form you file.
Ask what the office needs for that lot and that task. It may ask for a site plan, driveway point, road name, parcel record, or address letter. Alachua and Marion counties show two local paths, while another county may arrange the work in a different way.
Once the address is set, follow the local rules for posting it by the road. Keep the address letter or email with the permit, utility, and closing papers.
